Discharge Summary From Referring Facility
A discharge summary from the referring treatment center is not required but is appreciated. These discharge summaries can be mailed or faxed to the House.

Resident Rules

  1. The cardinal rule is that there shall be no drinking or taking of un-prescribed drugs by residents. A person who is taking un-prescribed drugs or drinking will be immediately discharged.
  2. The Caldwell House Director and staff have the authority to ask any resident who they suspect may be taking drugs/alcohol to take a drug test.  Failure to take this test will be grounds for dismissal.
  3. All residents shall be in the house by 10:00PM Sunday through Thursday.  Friday and Saturday, residents shall be in the house by 12:00 midnight. No resident shall be away overnight unless he has made arrangements with the house manager in advance.
  4. A resident must be at the Caldwell House for 30 days before being eligible for a pass.  All passes must be arranged 24 hours in advance and arrangements made by the resident, in person with a member of the staff to have assigned chores taken care of.  The administrator may grant an exception to this rule if unusual circumstances justify such an exception.  Being a resident of the house does not mean you automatically receive a pass.  You must show that you are responsible enough to be granted a pass.
  5. An important component of a resident's rehabilitation is steady employment.  It is expected that all residents will make an earnest effort to find a job.
  6. All resident's are required to attend all Caldwell House sponsored meetings. 

Finances

  1. Upon applicant's first interview, and if accepted, arrangements must specifically be made for payment either by the applicant or some responsible other person until employment can be obtained.
  2. Upon acceptance, four (4) weeks payment must be made prior to admission to the house.
  3. Late payment may constitute possible dismissal from the Caldwell House, except in rare circumstances and upon executive board approval. Note: If a resident sees that late payment will be necessary, they must first initiate contact with the director for arrangements to be made.
  4. Within one week of arrival at the Caldwell House, each resident will be registered with at least one of the following:  Employment Security Commission, Vocational Rehabilitation, one of the Temporary Services  and be actively seeking employment
  5. The cost of Room and Board is $120 a week and must be paid on time. No rent shall be refunded.
